Lifescore Foundation was founded in 2019 by Beyonca Johnson, a visionary leader committed to transforming the cycles of systemic inequity that impact underserved youth and families across Chicago.
Born out of lived experience, Lifescore was created as a solution at the intersection of healing, leadership, and opportunity.
After witnessing firsthand how poverty, injustice, emotional trauma, and systemic neglect trap generations in survival mode, Beyonca set out to build what she wished had existed for her: a space where people could break cycles, reclaim their futures, and lead lives rooted in healing and empowerment.
Lifescore’s early work focused on equipping youth with workforce training, mentorship, leadership development, emotional intelligence, and violence prevention tools. From the beginning, our belief has been simple but powerful: access to emotional resilience, economic opportunity, and holistic support is the foundation for community transformation.
Founding Vision
As the needs of our community deepened, so did our vision. Lifescore expanded its programs to include family empowerment, trauma healing, social-emotional education, and holistic wellness, launching initiatives like ROOTS for parents and Holistic Wellness and Workforce Pathways for youth and families.

Through strong partnerships with organizations such as ICJIA, CPS, IDHS Teen REACH, IYIP, RYDS, Think Outside Da Block, Playstreets, Chatham Academy, Ignite, NYAP, Nelmar, Acclivus, Third Circle, and The Healing Academy, Lifescore has evolved into a trusted resource for systemic change and transformational leadership.

As the official Convener for Chatham, Lifescore now leads regional violence prevention efforts, coordinates provider collaboration, facilitates community-driven solutions, and strengthens the regional ecosystem for safety, healing, and opportunity.
This role reflects the trust our community has placed in us and the responsibility we hold to create lasting, systemic transformation.
Today, Lifescore Foundation stands as a powerful resource for youth, families, and communities seeking not only to survive—but to heal, to lead, and to thrive.
